A Proof of Claim form is available on the court’s web site, under Forms > Official Bankruptcy Forms, Proof of Claim.

STEP 1 Choose Bankruptcy from main menu
STEP 2 Choose Claim Filing category
STEP 3 Enter the case number and name of creditor; click [NEXT]
STEP 4.a Using the Select a Creditor pull-down, search for the creditor
STEP 4.b If the creditor is found, highlight the creditor’s name and click [NEXT]
TIP – A pop screen displays the name and address of the creditor, if correct click [YES] and skip to STEP 5
STEP 4.c If the creditor is not found, click the Add Creditor link. Click here for instructions on adding creditors.
STEP 4.d If the creditor appears on the list, but with an incorrect address, you must add the creditor and the correct address. Click here for instructions on adding a creditor.
STEP 5 Click here for instructions on filing an Amended Proof of Claim; otherwise skip to STEP 6.
STEP 6 Enter the amount in the appropriate field(s); click [NEXT]
TIP 1 – You may enter a information in the Description or Remarks fields, but it is not mandatory.
TIP 2 – By pressing the tab key through the fields, the Total field will calculate
STEP 7 Upload PDF file and any attachments; click [NEXT]
IMPORTANT REMINDER – The court receives many proofs of claim and attachments that contain Personal Data Identifiers (PDI). Please be sure to redact all PDI’s prior to uploading documents.
